Welcome to Fantastic 4rms. This site was created to provide you with an opportunity to further explore Google Forms and create your own classroom applications.
You are able to choose any of the tasks to complete in any order. I would suggest if you haven't used forms before to begin watching the video to the left. You can click on the video to move to the segment you desire. When you complete a task, submit it by clicking on the Submit Artifact icon and earn your certification of completion. |

Explore Google Forms Create a quiz with at least 5 question that includes multiple choice and or short answer questions. Also include the following: Questions for student name and class period. Explore adding Visuals Create a quiz with at least 5 question that includes multiple choice and or short answer questions. This could be a quiz that you have already created and continue to modify. Also include the following: Questions for student name and class period. This helps when assessing the quiz. Complete the following tasks: 1. Insert an image or video into a google form and add questions underneath that could be answered based on the visual. (Youtube videos) 2. Insert an image or video into a self guided form where the image or video would provide additional instructions or reteaching to assist the student in answering the original question. Explore AutoCrat and Document Merge Using a Google Form that you use for collecting information or as a rubric complete the following. Create a Google Document that you will use as a template for information imported from your Google Form Go to the Google Sheet connected to your Google Form and add the AutoCrat Add-On Complete one of the following: 1. Create individualized feedback for students by merging data from the rubric sheet to the google doc template 2. Self Grade/FlubarooTask 2
Explore the self grading Add-on Flubaroo After you have created a quiz, or using a pre-existing quiz with multiple choice and or short answer questions, use Flubaroo to self grade the exit ticket. You will need to go to the spreadsheet/form responses in order to complete this task. You will need to complete the quiz a few times to populate it with responses and create an answer key. Explore Learning Adventures Create a form that collects student responses to tasks or answers clues and then links to the next clue or task. Complete the following: 1. Create a form that uses data validation to verify the accuracy of student response before they can move on to the next question or task. 2. Add a link to new form or document that includes the next task or clue by adding the URL of this doc/form in the Submission Confirmation Page 3. Connect the forms to a QR code or GoogleMyMaps to create an activity Bonus: Create a Lesson Plan where you complete the above tasks with real content and write a reflection about how well the lesson went. Self Guided ReviewTask 3
Explore the Data Validation Complete 1 of the following tasks: 1. Create a self guided review that uses short answer and multiple choice questions. Include data validation to redirect or check student responses. 2. Create a form that includes data validation that will only allow participants access to the form, or be able to link to another document if they get the correct answer. (See form that links to this PD page, or Amazing Race as example) 3. Create a form that incorporates data validation and links correct and incorrect responses to visual confirmation of whether response is correct. Explore Rubric Creation Create a form that is a digital representation of a rubric you already use. Include student name and hour in the form. Complete one of the following: 1. Create a form that uses a linear scale and comments to assess student learning. 2. Create a form that uses checkboxes that correspond to the criteria of your rubric. 3. Create a form that uses multiple choice grid where the rows correspond to the categories and the columns represent the levels ie. Basic, Developing, Proficient, Advanced.
